As a Hospice Speech Language Pathologist, you will plan, organize, and deliver compassionate, specialized therapy services focused on comfort, communication, and quality of life. Guided by our values—trustworthy, capable, compassionate, proactive, and called—you will help patients and families navigate meaningful goals at the end of life. Every encounter is an opportunity to make a meaningful difference. Essential Functions Patient Care Conduct evaluations and assess the home environment to determine equipment and speech/language needs. Identify functional speech and communication deficits; establish an individualized plan of care to improve or maintain essential function. Administer speech therapy programs using specialized techniques and equipment. Recommend communication devices or assistive aides as needed. Instruct patients, families, caregivers, and agency personnel on treatment plans, communication strategies, and safe care techniques. Collaborate closely with referring physicians and the interdisciplinary team to ensure comprehensive, coordinated care. Communication & Documentation Prepare and submit clinical notes in alignment with organizational policy. Communicate ongoing patient needs, changes, and progress to physicians, caregivers, and team members. Participate in interdisciplinary group (IDG) meetings, in-services, and other required team activities to support holistic care planning.
