As a Certified Occupational Therapy Assistant, you will provide compassionate, hands-on occupational therapy services under the direction of a Registered Occupational Therapist. You will help patients maintain dignity, independence, and comfort while supporting meaningful function at the end of life. Essential Functions Patient Care Participate in ongoing evaluation of a patient’s functional status—including muscle function, endurance, coordination, communication abilities, self-care skills, and work capacity—as delegated by the Occupational Therapist. Assist in assessing the home environment for safety hazards or barriers to independence. Teach and retrain patients in daily activities lost due to illness or injury, in alignment with organization policy. Follow the established treatment plan and goals created by the Occupational Therapist; document patient response and progress. Identify patient or caregiver needs for additional home care services; consult with the supervising OT and assist with referrals as appropriate. Participate in educating caregivers and interdisciplinary team members on patient treatment regimens as delegated. Communication & Documentation Maintain appropriate clinical records and promptly report any changes in patient condition to the supervising OT. Submit accurate, timely documentation accor
